Grant Funding Request Intake Form

The Development Team is committed to helping programs identify and pursue grant funding to expand opportunities and enhance support for people with intellectual and developmental disabilities. If your department has a need, project, program enhancement, equipment request, or new idea that could benefit the people we support, we encourage you to complete the Grant Funding Request Intake Form. Submitting requests early helps the Development and Grants Manager to explore funding opportunities, assess eligibility, and develop the strongest possible proposals.
